It is our office policy to require in-person office visits at least quarterly for all controlled substance prescriptions.

​Paradise Valley Family Medicine does not provide COVID-19 vaccinations.


MASK REQUIREMENT

Masks are not required for in-office appointments at this time.  You are welcome to wear one if you are more comfortable doing so.

Paradise Valley Family Medicine is not a weight loss clinic.  Although we will try to assist you in your weight loss journey, we do not prescribe medications off-label such as ozempic and will not obtain prior authorizations for weight loss medications.​
