INSURANCE

Paradise Valley Family Medicine participates with most major health insurance, health maintenance programs (HMOs) and Medicare advantage plans (MAP).  We will file other insurances on your behalf; however, you are responsible for payment.

We recommend you become familiar with your health insurance policy, its coverage, limitations, payments and regulations concerning visits, referrals and preauthorizations.  Because all insurance contracts are between the patient and the insurance company, you are ultimately responsible for payment.

TELEPHONE INQUIRIES

We encourage you to call during office hours with medical concerns.  Our staff has been trained to answer many of your questions and will screen the call for urgency.  Please give them as much information as possible.  If they are not able to help you, one of the physicians or medical assistants will return your call at his or her earliest convenience.


PATIENT PORTAL


Although we encourage you to communicate with your provider through the patient portal.  This is not to be used in lieu of an office visit.  Requests for new medications or advice to an undiagnosed condition will require an office visit.

 
PRESCRIPTIONS

Prescription refills should be requested 72 hours in advance through your pharmacy.  Your pharmacy will contact us for refill orders.  Prescriptions for antibiotics, pain medicines or scheduled medications generally require an office visit.


In office appointments are required at least yearly for regularly scheduled medications and quarterly for opioid, amphetamines and benzodiazepines. Your provider may request more frequent visits based on your condition and medication requirement.  This is following standard of care guidelines in place by the medical community and Federal and State prescribing guidelines.



CONTROLLED MEDICATIONS


Controlled medications such as opioids, benzodiazepines and amphetamines require in office appointments quarterly (every 3 months).  Additionally, you may be required to take a random urine drug screen (UDS) as recommended by the DEA following standard of care guidelines.  Also, you will be required to sign a Controlled Substance Agreement.



OFFICE HOURS

Monday - Thursday:           8:00 a.m. to 5:00 p.m.

Friday:                                   8:00 a.m. to noon

If you need to reach a physician after hours, call our office at 602-494-5155 to reach the answering service.  If your problem is not urgent, please contact us during office hours the next business day.  Medication refills will not be made by the provider on call.


APPOINTMENTS

Routine well visits will be scheduled with your primary care physician.  Please call several weeks in advance for routine physicals and checkups, so we may better accommodate your scheduling needs.

If you have an insurance/address change, please arrive 15 minutes prior to your scheduled appointment to complete a patient information form.  Always remember to bring your insurance card with you.  If you are a new patient to Paradise Valley Family Medicine, please bring a copy of your medical records with you or the name and address of your previous physician.

If you are unable to keep an appointment, please call and let us know at least 24 hours in advance.

NOTE:  No show or late cancellations may result in a $25 cancellation fee.
